Anticipatory Care Planning sessions

Awareness sessions on Anticipatory Care Planning (ACP) and power of attorney will be held on Monday 19 December in the boardroom at Udston Hospital in Hamilton.
There will be a short presentation and updates regarding the new national ACP, which is to be launched in January, and also the ACP app for mobile phones.
Stuart McDonald from Equals Advocacy will attend to go over power of attorney and legal issues concerning patients and their family and carers.
Sessions are at 9.30am, 10.30am, 11.30am, 1pm, 2pm, and 3pm.
